Welcome to the Online COVID-19 Self-Screening Tool for Employees, Physicians, Residents and Approved Learners. As part of our commitment to maintain a safe and healthy workplace, you are required to complete an online self-screening tool prior to every admittance to the hospital site.

Online Screening Tool Consent

Prior to completing the tool, you are asked to agree and/or consent to the following:

  • You are an HHS employee, physician, resident or approved learner, patient transport staff & approved contractors;
  • Your answers to the screening questions are honest and reflect your best knowledge;
  • You understand that the results of your screening will be shared with HHS Employee Health Services for review and follow up, as appropriate;
  • This screening system is secure, but no electronic system is entirely without risk;
  • You will be asked to provide your email address so that your results can be emailed to you. Communication by email carries inherent privacy and security risks that your personal health information may be intercepted or unintentionally disclosed. That risk is increased if you choose to communicate using a non-HHS email address, as your information may not be encrypted.
  • NOTE: Patient transport staff/approved contractors will not be contacted by HHS Employee Health Services for review of screening results. Please contact your employer/public health.
